LPN - Office / Patient Coordinator

Position Scope:
To ensure effective patient care coordination/human resource/office coordination needs for the services working with the company's personnel, and through liaison with other organizations and individuals also providing care to the patient/client.
Ongoing reviews the utilization and progress of the patient with the RN, supervisor and attending physician as necessary.
Lines of authority and reporting responsibilities: Report to the Director of Nursing, Clinical Manager, Nurse Supervisor, Administrator.
Position Qualifications:
Graduate of high school or with some years of high school education (no Diploma required). Evidence of knowledge of home care environment; ability to work under stress, and to take rapid actions.
Verbal and written communication skills.
Good organizational skills.
Physical requirements:

MENTAL REQUIREMENTS

Performance, abilities and standards:
Medical records filing (electronic, paper).
Human resource records filing (electronic, paper).
Basic computer data entry/typing, answering/making phone calls.
Institute a set of checkpoints to make sure that the services were provided/customer satisfaction surveys.
Participates in human resources onboarding/clearance procedures.
Participates in facility activities directed to implementation of safety management program, security plan, utility plan, emergency preparedness plan, etc.
Is responsible for confidentiality issues.
Ensures HIPAA guidelines and procedures are maintained.
Understands the nature and type of the patient/client population serviced.
The LPN will be aware of the responsibilities of all organizations and individuals involved in patients/clients care/service including the coverage for the services rendered.
The LPN will participate in education conferences, meetings, in-services and training for policy and procedure modifications, emergency response and preparedness plan, organization planning, quality assurance and company activities improvement, etc.
The LPN gives accurate information to clients, clients families and other professionals involved in patients care/service.
Complies with all applicable policies and procedures, federal and state rules, regulations and laws in effect.
Participate in personal growth and development.
Documents/files all communications and the communication notes from the patient's chart.
- Reimbursement is subject to applicant's work experience.
Required Qualifications:
Active Licensed Practical Nurse with a current unencumbered license in the state of Florida
Have and maintain current CPR certification
Possess a valid and current drivers license and auto insurance
Experience with Office products including, but not limited to, Microsoft Office, Excel, Outlook and Word document. Willingness to learn other office and software programs
Electronic Medical Record system familiarity
- On-call rotation participation as indicated by business needs
Preferred Qualifications:
Bilingual STRONGLY preferred (English and Spanish)
Tech Savy highly desired
Education:
Graduate of a school of Practical Nursing
