About the job SUPPORT WORKER
Working as part of a 2-person crew to transport medically fit and mobile patients home, as appropriate, using suitable British Red Cross vehicles.
> To carry out a needs and environmental risk assessment on arrival at the patients home
> To provide a high level of practical support.
> To work closely with the other Team Support Workers in the service, ensuring a high-quality service delivery.
> To assess and plan the individual aftercare needs of the patient, if appropriate.
> To work within a team to help with the maintenance of the British Red Cross vehicles carrying out daily checks and ensuring the vehicles are performing satisfactorily.
Ø To liaise with health professionals at the Darent Valley hospital responding to referrals in a timely and professional manner.
> To assist with publicising the Darent Valley Assisted Discharge Service to hospital teams.
> To uphold the Fundamental Principles of the Red Cross and Red Crescent Movement and to work within the Red Cross Equal Opportunities Policy and other Society policies.
> Ensure high quality and safe services (meeting requirements of BRCs quality standards framework, and health and safety, hygiene/infection control, and safeguarding procedures)
> Adhere to person centred assessment, risk management, support planning and reviewing procedures.
