As a key member of the Interdisciplinary Team (IDT), you will play a central role in assessing participant needs, implementing individualized care plans, and providing direct nursing care across clinical, in-home, and community-based environments. Key Responsibilities

• Participate actively in the Interdisciplinary Team (IDT), contributing assessment insights, care planning recommendations, and ongoing care coordination.

• Conduct comprehensive nursing assessments (physical, psychosocial, and behavioral) in a range of settings including the clinic, home, day center, virtual visits, and long-term care facilities.

• Collaborate with medical providers to deliver personalized care to a panel of PACE participants in alignment with care plans.

• Administer medications and therapeutic interventions accurately, responding promptly to physician orders and participant needs.

• Independently manage triage in outpatient settings and escalate care appropriately in collaboration with medical providers.

• Provide case management support during transitions of care, maintaining close communication with hospital and long-term care facility case managers.

• Educate participants, caregivers, and team members to support the implementation and success of personalized care plans.

• Participate in a rotating on-call schedule, providing after-hours triage support via phone.

Schedule & Location

• Full-time, on-site position

• Monday through Friday, daytime hours

• Based in Downtown Los Angeles
