As a key member of the Interdisciplinary Team (IDT), you will play a central role in assessing participant needs, implementing individualized care plans, and providing direct nursing care across clinical, in-home, and community-based environments. Key Responsibilities

Participate actively in the Interdisciplinary Team (IDT), contributing assessment insights, care planning recommendations, and ongoing care coordination.

Conduct comprehensive nursing assessments (physical, psychosocial, and behavioral) in a range of settings including the clinic, home, day center, virtual visits, and long-term care facilities.

Collaborate with medical providers to deliver personalized care to a panel of PACE participants in alignment with care plans.

Administer medications and therapeutic interventions accurately, responding promptly to physician orders and participant needs.

Independently manage triage in outpatient settings and escalate care appropriately in collaboration with medical providers.

Provide case management support during transitions of care, maintaining close communication with hospital and long-term care facility case managers.

Educate participants, caregivers, and team members to support the implementation and success of personalized care plans.

Participate in a rotating on-call schedule, providing after-hours triage support via phone.
