Position Overview

Position Summary:
Seeking a licensed Nurse Practitioner to join our primary care team, providing direct patient care including physical exams, chronic disease management, acute illness evaluation, patient education, and care coordination. Ideal candidates are patient-centered, proactive, and enthusiastic about integrating into a multidisciplinary setting.

Key Responsibilities

  • Perform evaluations, diagnostics, and physical exams for patients of all ages (pediatric through geriatric)

  • Manage chronic conditions (e.g., diabetes, hypertension, asthma) using evidenced-based protocols

  • Diagnose and treat acute illnesses (infections, injuries, etc.)

  • Order and interpret labs, diagnostics, and imaging studies

  • Prescribe medications (including controlled substances), following Arizona PN Care &az Controlled Substances regulations

  • Provide health promotion, disease prevention, and patient education

  • Collaborate with physicians, specialists, RN staff, and care coordinators

  • Maintain accurate and timely electronic health record (EHR) documentation

  • Participate in quality improvement initiatives and performance metrics tracking

  • Potential supervisory duties over support staff and mentorship of students

‍️ Qualifications

Education & Licensure:

  • Masters or Doctoral degree in Nursing (MSN or DNP) from an accredited NP program

  • Board-certified by a recognized body (e.g., AANP, ANCC)

  • Active, unrestricted Arizona NP license (or eligibility via portability)

  • Federal DEA license (AZ DEA # required)

Experience & Skills:

  • Minimum of 12years clinical NP experience (primary care/family medicine preferred)

  • Competency in diagnosing/treating common primary care conditions

  • Familiarity with chronic disease protocols (diabetes, high blood pressure, lipids)

  • Proficient in EHR workflowexperience with Epic, Cerner, Athenahealth is a plus

  • Strong interpersonal, communication, and cultural competency skillsespecially for diverse patient populations, including Hispanic and Native American communities

  • Valid CPR/BLS certification
