Outreach Family Support Worker
Job Description:
We are working closely alongside with a local authority in Liverpool to assist with the appointment of a Outreach Family Support Worker on a 3-months contract, highly likely to be extended at clients discretion. Please apply with your CV for immediate consideration.
Rate of Pay: £16.50 - £21.59 per hour
Summary:
The Outreach Family Support Worker (OFSW) provides high-quality, intensive support to children (ages 0–25*) and their families using a "whole family" approach. Working at Level 2 and 3 of the Early Help framework, you will take full case responsibility to prevent the need for statutory intervention. Your goal is to empower families, improve outcomes, and ensure they can successfully transition to or access universal services. You will act as a Lead Professional, coordinating "Team Around the Family" (TAF) meetings and delivering bespoke support packages in the community and family homes.
Responsibilities:
- Act as the primary case holder for children with multiple support needs, including those "stepping down" from statutory social care.
- Complete comprehensive Family Help assessments and formulate TAF plans designed to reduce risk. Lead the coordination and regular review of these plans.
- Deliver child-centered, targeted support and bespoke programmes to vulnerable children, including adolescents dealing with complex emotional or mental health challenges.
- Promote the safeguarding of children in accordance with statutory legislation and City Council policies, remaining alert to changing circumstances inside and outside the home.
- Maintain precise electronic records, including chronologies and general notes, and produce detailed professional reports within strict deadlines.
Qualifications:
- A relevant Level 3 or Level 4 qualification in Childcare, Social Care, Youth Work, or a related field is essential.
Essentials:
- Substantial experience working directly with children and families in a community, outreach, or home-based setting.
- Understanding of the SEND Code of Practice and the levels of intervention within Children's Services.
- A commitment to maintaining up-to-date knowledge of the legal frameworks and attending regular supervision with the Consultant Social Worker.
