Social Work Associate

Responsibilities

• Conduct mean testing for all relevant patients
• Gather and document socio-financial information of patients and family members to facilitate intervention by medical social worker/ counsellor.
• Provide administrative support to MSW(s)/ Counsellors and administration of client cases.
• Assist in follow up phone calls and liaison work with family members on gathering of necessary documents to facilitate completion of means test, inpatient admission paperwork and financial assistance.
• Update and maintain training manual for new staff.
• Provide basic information and referral services (Inclusive of outreach services for Bereavement Care Programme & answering any phone calls or walk-in enquiries).
• Assist in Departmental programmes and any other duties assigned by Supervisor/ Head of Department.
• Participates in in-service education programme and follow-up to ensure optimum transfer of learning.

Requirements

• Diploma in Social Work, Social Services, community development related disciplines.
• Preferably with 1-2 years of working experience in health and/or social service sector.
• Bilingual and ability to communicate in local dialects preferred.
• Passion to serve and provide good patient care.
• Good interpersonal skills and able to work independently.
• Flexible, adaptable and able to work effectively in varied setting.
• Good in analytical and interviewing skills.
• With aspiration to pursue social work degree, preferably.
