About the job PCN Clinical Pharmacist (Remote)

We have several exciting FULL TIME REMOTE opportunities for Prescribing Pharmacists, Non-Prescribing Clinical Pharmacists, and PCN Clinical Pharmacists.
We are seeking experienced Clinical Pharmacists, PCN Pharmacists, or Practice Pharmacists who are passionate about making a significant impact on patients' lives.

Role Requirements Essential:
Experience as a Clinical Pharmacist, Remote Pharmacist, or Practice Pharmacist.
Previous experience in general practice or as a GP Pharmacist.
Role Requirements Desirable:
Independent prescribing qualification (essential for prescribing roles, not for non-prescribing roles).
Qualification or enrollment in the CPPE Clinical Pharmacist Training pathway (offered for PCN pharmacists to enhance clinical skills).
Strong clinical leadership, communication, and interpersonal skills.
Good understanding of the role of a PCN Pharmacist.
Role Responsibilities:
Improving health outcomes for long-term conditions.
Providing specialist pharmaceutical advice.
Reviewing acute medication requests.
Completing discharge and clinic letters.
Conducting structured medication reviews as a PCN Pharmacist.
Advising and supporting patients with self-limiting conditions.
Supporting practice achievements of key Impact and Investment Fund (IIF) targets and QOF targets.
