Job Location: Clark Outsourcing - Building 35 Philexcel Business Park, Clark Pampanga

Quick Rundown:

We are seeking a dedicated and detail-oriented Client Care Representative to support client care coordination and scheduling operations. This role is responsible for matching caregivers or nurses with clients based on care needs, managing schedules, and maintaining positive relationships with clients and care staff. The ideal candidate is highly organized, communicates effectively, and can manage multiple priorities in a fast-paced environment.

What You’ll Do:

Care Coordination:

  • Assess client needs to assign appropriate caregivers or nurses

  • Match caregivers or nurses with clients based on compatibility, skills, and availability

  • Ensure timely and appropriate caregiver replacements when necessary

  • Relationship Management:

  • Develop and maintain positive relationships with clients and caregivers

  • Review nurse notes for quality assurance

  • Coordinate with the RN Care Manager regarding client conditions and care needs

  • Provide updates and feedback to the nursing team regarding client status

Documentation and Scheduling:

  • Assist with client documentation and accurate data entry

  • Follow up on physician orders, authorizations, and related documentation

  • Create and maintain schedules for clients, nurses, and caregivers

  • Adjust schedules using Alora and ABS systems

  • Maintain tracking sheets

Attendance Monitoring:

  • Monitor caregiver and nurse attendance

  • Address scheduling conflicts and coverage needs

  • Follow up on clock-in/clock-out discrepancies

  • Provide basic technical support and user guidance when needed

Communication and Call Handling:

  • Handle new patient intake calls and enter information into the ABS system

  • Answer incoming calls professionally and route them appropriately

  • Provide accurate information and assistance to callers while maintaining client confidentiality

Client Communication and Support:

  • Provide updates to clients regarding caregiver or nurse schedules

  • Address client inquiries, concerns, and service requests

  • Document changes and maintain accurate client records

  • Conduct follow-ups to ensure client satisfaction


What We’re Looking For:

  • Experience in scheduling, coordination, or a similar role, preferably in healthcare or home care

  • Experience working in a virtual or remote support role is a plus

  • Proficiency in computer software, scheduling platforms, and administrative tools

  • Familiarity with healthcare regulations and compliance standards is a plus

  • Strong organizational skills with high attention to detail

  • Excellent communication and interpersonal skills

  • Compassion and empathy when working with clients and caregivers

  • Strong problem-solving abilities, particularly in complex interpersonal situations

  • Comfortable handling difficult conversations over the phone

  • Managing multiple communication channels, including messaging, emails, and other correspondence

  • Must be willing to work on weekends, holidays and have a flexible schedule

  • Ability to work independently while managing multiple priorities

  • Team-oriented with a willingness to contribute ideas and feedback
