Primary Care Clinic LPN & Medication Authorization Nurse
Location: Clifton‑Fine Region, NY Department: Primary Care Clinic Schedule: Full‑Time Reports To: Practice Manager & Supervising Provider
Clifton‑Fine Hospital's Primary Care Clinic is seeking a skilled Licensed Practical Nurse (LPN) to join our primary care team. This role blends hands‑on clinical care with medication authorization and refill management, ensuring patients receive timely, safe, and coordinated care. What You'll Do
Clinical Care
- Administer vaccines and medications per standing orders and provider direction
- Perform POC testing (A1c, glucose, strep, flu, UA, pregnancy tests)
- Assist with procedures (wound care, ear lavage, nebulizers, EKGs)
- Provide patient education and chronic disease support
Medication Authorization & Refill Management
- Process medication refills per protocol
- Complete medication prior authorizations using payer portals
- Communicate with pharmacies and troubleshoot coverage issues
- Track PA status and ensure timely follow‑up with patients and providers
Care Coordination Support
- Support chronic disease workflows (hypertension, diabetes, COPD, CHF)
- Conduct nurse visits (BP checks, inhaler teaching, glucose checks)
- Assist with care gap closure and follow‑up outreach
- Collaborate with Care Coordinators, Referral Coordinators, and providers
What You Need
- Current NYS LPN license in good standing
- Experience in primary care or outpatient settings preferred
- Strong understanding of medication workflows and prior authorizations
- Excellent communication, organization, and patient‑centered care skills
- Comfort working in a rural, team‑based environm
