Overview:
We are seeking a highly skilled and patient-focused Adult-Gerontology Nurse Practitioner (AGNP) to provide comprehensive healthcare services for adolescent, adult, and elderly populations. Depending on specialty focus, the AGNP may practice in primary care (AGPCNP) or acute care (AGACNP) settings, delivering preventive care, chronic disease management, acute illness treatment, and age-specific healthcare services.
This role requires advanced clinical expertise, strong diagnostic reasoning, and the ability to manage medically complex adult and geriatric patients across diverse healthcare settings.
Key Responsibilities:
- Clinical Assessment & Patient Care
- Conduct comprehensive physical exams, health histories, and age-appropriate assessments
- Diagnose and manage acute illnesses, chronic diseases, and age-related health conditions
- Develop individualized treatment plans for adult and geriatric patients
- Order, interpret, and evaluate diagnostic tests, imaging, and laboratory results
- Prescribe medications and therapies within scope of practice
- Perform preventive screenings, wellness visits, and health promotion initiatives
- Manage chronic diseases such as hypertension, diabetes, COPD, cardiovascular disease, and dementia
- Provide palliative, rehabilitative, or specialty support depending on patient needs

AGPCNP (Primary Care Focus):
- Preventive care
- Outpatient chronic disease management
- Geriatric wellness
- Health maintenance
- AGACNP (Acute Care Focus):
- Hospital-based acute care
- Critical illness management
- Inpatient specialty medicine
- Complex procedural support
Care Coordination & Collaboration
- Collaborate with physicians, specialists, nurses, social workers, and interdisciplinary teams
- Coordinate referrals and transitions of care
- Educate patients and families regarding disease prevention and management
- Participate in multidisciplinary rounds or outpatient care planning
Documentation & Compliance
- Maintain detailed and accurate EMR documentation
- Ensure compliance with regulatory standards and clinical protocols
- Participate in quality improvement initiatives and patient outcome monitoring
- Maintain licensure, certification, and continuing education requirements
Requirements:
- Master's or Doctoral degree in Nursing (MSN, DNP) with Adult-Gerontology specialization
- Active Registered Nurse (RN) license
- Active Adult-Gerontology NP certification (AGPCNP or AGACNP)
- DEA registration and prescriptive authority (where applicable)
- Strong diagnostic, clinical management, and patient education skills
- Expertise in adult and geriatric medicine
- Ability to work independently and collaboratively
- Proficiency in EMR systems and advanced documentation
Preferred (Nice-to-Have):
- Experience in primary care, geriatrics, hospital medicine, cardiology, or specialty care
- Experience with elderly populations and chronic disease management
- ACLS/BLS certifications for acute care settings
- Experience in palliative care, long-term care, or transitional care
- Telehealth experience
- Leadership or advanced specialty certification
