Competitive Salary: $75,000 $95,000 per year (DOE)

Key Responsibilities
-
Deliver home visits in assigned territories (Phoenix & surrounding areas)
-
Perform admissions, evaluations, recertifications, and individualized care plans
-
Administer medications, wound care, and treatments
-
Monitor client health and report changes to supervisors
-
Ensure accurate documentation via EMR and mobile devices
-
Coordinate care and maintain strong communication with physicians and families
-
Provide case management and follow-up support
Qualifications
-
Active Registered Nurse (RN) license in Arizona
-
Minimum 1 year of recent clinical experience (medical/surgical)
-
Prior home health experience preferred (training provided if not)
-
Graduate of an accredited nursing program
-
Strong time management, critical thinking, and interpersonal skills

Comfort with EMR and mobile documentation tools

Reliable transportation to visit clients across the coverage area

Proficiency in administering medications, wound care, and medical procedures

Ability to conduct accurate patient assessments and implement care plans

Experience with electronic medical records (EMR) and mobile charting tools

Knowledge of OASIS documentation (preferred but not required)
