Vivo HealthStaff is recruiting for a Staff Physician in San Jose, California. This is a permanent position with salary and benefits. We are seeking a Staff Physician to join a multidisciplinary geriatric program in San Francisco. This full-time position involves providing comprehensive primary care services to older adult patients across a variety of care settings including clinic, hospital, nursing home, and home visits. **Position Summary:** Under the administrative supervision of the Supervising Physician, the Staff Physician is responsible for delivering medical care to patients enrolled in the program’s comprehensive senior care services. The physician works as part of a collaborative, interdisciplinary team providing compassionate, patient-centered care to a medically complex and culturally diverse population. **Key Responsibilities:** - Conduct complete histories and physical exams for all new patient referrals and enrollees - Develop individualized treatment plans in collaboration with the Intake and Assessment Team - Perform interval physical exams and monitor medical changes, with a minimum of biannual exams per patient - Provide primary care across settings: outpatient clinic, hospital, nursing home, and patient homes - Consult and co-manage cases with nurse practitioners and nursing staff as needed - Coordinate care with the internal medical team, including dentistry, podiatry, audiology, and speech therapy - Authorize referrals to medical and surgical specialists when necessary - Serve as attending or consult with the Hospitalist when patients are hospitalized - Provide monthly visits for patients residing in skilled nursing facilities - Participate in the on-call physician rotation for direct patient care - Complete timely documentation and participate in staff meetings and in-service training - Educate patients, families, and community members about the program as needed **Qualifications:** - MD or DO with a current license to practice medicine in the State of California - Board Certified or Board Eligible in Internal Medicine or Family Medicine - Subspecialty training or a minimum of one (1) year of experience working with elderly or medically complex adult patients required - Current DEA Certificate - Current BLS or ACLS certification - Commitment to working within an interdisciplinary, team-based care model - Strong communication skills and ability to work with a multicultural patient population - Bilingual ability preferred for specific assignments **Compensation & Benefits:** - Competitive salary and benefits package - Medical, Dental, and Vision coverage - 403(b) Retirement Savings Plan - Life and AD&D Insurance - Flexible Spending Account (FSA), Commuter Benefits, and Employee Assistance Program (EAP) - Pet Insurance and additional employee discounts - Paid holidays, vacation, sick leave, and long-term disability insurance
