Key Responsibilities

• Conduct skilled nursing assessments in the home and contribute to interdisciplinary care plans

• Deliver and document care aligned with physician orders and patient needs

• Administer medications, perform ordered treatments, and ensure a safe home environment

• Educate participants and their families on disease management, self-care, and preventative health

• Coordinate care transitions between hospitals, skilled nursing facilities, and home

• Track and manage visit schedules in coordination with the Home Health Manager

Qualifications

• Graduate of an accredited School of Nursing

• Active, unrestricted RN license in California

• 1+ year of nursing experience with elderly or frail populations (home health experience preferred)

• Reliable transportation for local travel

• Bachelors degree preferred
