In compliance with the Data Privacy Act of 2012 (R.A. 10173), Eskwelabs Technology Ventures Inc. ("Eskwelabs") seeks your consent to collect, use, and store the personal information and sensitive personal information (collectively, "personal data") disclosed by you in this form and other required forms for your mentorship application. Your personal data will be used for the following purposes:

1. Processing your application at Eskwelabs, Inc.

2. Reporting demographic information of applicants internally to the team without disclosing personal information.

Your personal data will not be used for any other purpose than those stated above. Access to your personal data is restricted to authorized personnel, such as project managers and hiring managers for sprints, bootcamps, and internal hiring needs.
