Statement of Work. The Contractor shall provide the Contractor Personnel Services and staff, and otherwise do all things
necessary for or incidental to the performance of Contractor's Services, as set forth below
A. Optometry Services. The Contractor shall perform Services within the practice area of Optometry, including, but not
limited to, the following optometry services:
Primary vision care;
Examination of Patients' eyes to diagnose vision problems and eye diseases;
Tests of Patients' visual acuity, depth, and color perception, and ability to focus and coordinate the eyes:
Vision therapy and low-vision rehabilitation;
Analysis of test results and development of a treatment plan;
Administration of drugs to Patients to aid in the diagnosis of vision problems and prescribing of drugs to treat some eye
diseases;
Issuance of prescriptions for eyeglasses and contact lenses: and
Provision of preoperative and post-operative care to cataract Patients.
B. Contractor Responsibilities.

1. The Contractor shall conduct up to one (1) Optometry Clinic per month at the East Campus Clinic.
The Contractor may change Clinic schedule as agreed upon by both parties.
WSH may add additional Clinic hours as agreed upon by the Contractor.
Each Clinic shall last no longer than five (5) consecutive hours.
The Contractor shall schedule Clinics at least one (1) month in advance with the East Campus Clinic nurse.
WSH shall schedule Patient appointments with Contractor for each Clinic. c. Specific Contractor Qualifications and

C. Specific Contractor Qualifications and Licensing Requirements.
The Contractor shall hold a Doctorate of Optometry from an accredited Optometry school.
The Contractor shall be licensed as an Optometrist in the State of Washington, pursuant to Chapter 18.53 RCW -
Optometry, 18.54 RCW Optometry Board and Chapter 246-851 WAC Optometrist Rules.
