What Were Looking For:

We are seeking a Care Coordinator with proven experience in the health and social care sector, ideally within supported living services. You should have a Level 3 (or higher) qualification in Health & Social Care (or equivalent), and a strong understanding of CQC regulations, safeguarding, and person-centred care.

You'll be a confident leader, with excellent organisational and communication skills. The role requires someone who is flexible and able to work occasional evenings and weekends. Proficiency in IT, including Electronic Care Management Systems, is essential.

Experience or knowledge in Positive Behaviour Support (PBS), Trauma-Informed Care, Mental Health, Learning Disabilities, and behaviours that challenge would be a significant advantage.

Key Responsibilities:

  • Deliver person-centred care and support, ensuring the well-being of service users and helping them to achieve their individual goals.

  • Coordinate care plans, and collaborate with service users, families, and external agencies to provide tailored support.

  • Monitor the effectiveness of care plans and make adjustments as necessary.

  • Ensure compliance with CQC standards and safeguarding requirements.

  • Provide leadership and support to care staff, fostering a collaborative team environment.

  • Maintain accurate records using Electronic Care Management Systems.

  • Work with service users who may display challenging behaviours, applying PBS and Trauma-Informed Care approaches.

  • Flexible scheduling, including evening and weekend shifts.

What Youll Bring:

  • Proven experience as a Care Coordinator, ideally within supported living or similar settings.

  • A Level 3 (or higher) qualification in Health & Social Care, or an equivalent qualification.

  • Strong understanding of CQC regulations and person-centred care principles.

  • Excellent leadership, communication, and organisational skills.

  • Proficiency in IT and Electronic Care Management Systems.

  • Knowledge of Positive Behaviour Support (PBS), Mental Health, Learning Disabilities, and behaviours that challenge would be highly desirable.

  • A full, clean driving license and access to your own vehicle.
