YOUR RESPONSIBILITIES:
- Perform comprehensive physical assessments and review medical histories to accurately evaluate patients' conditions.
- Develop and customize treatment plans to meet the unique rehabilitation goals and needs of each patient.
- Implement therapeutic exercises and training programs aimed at improving mobility and overall functionality.
- Provide education to patients and their families on injury prevention, wellness strategies, and recovery processes.
- Track and document patients' progress toward rehabilitation goals, modifying treatment plans as needed.
- Collaborate with a multidisciplinary team of healthcare professionals to ensure well-rounded patient care.
- Maintain meticulous patient records and documentation in compliance with regulatory standards.

QUALIFICATIONS:
- Masters degree in Physical Therapy.
- At least 1 year of experience as a physical therapist in a home health setting.
- Valid state licensure to practice as a physical therapist, following professional standards.
- Current CPR certification is required during clinical employment.
- Valid driver's license with the capability to travel to various client locations
