KEY RESPONSIBILITIES:

• Conduct thorough physical assessments and analyze medical histories to accurately evaluate patients' conditions.
• Design and implement personalized treatment plans to address the distinct rehabilitation goals and needs of each patient.
• Execute therapeutic exercises and training programs to enhance mobility and overall functionality.
• Educate patients and their families on injury prevention, wellness strategies, and the recovery process.
• Monitor and document patients' progress towards rehabilitation goals, adjusting treatment plans as necessary.
• Collaborate with a multidisciplinary team of healthcare professionals to ensure comprehensive patient care.
• Maintain detailed patient records and documentation in compliance with regulatory standards.

QULIFICATIONS:

• Masters degree in Physical Therapy.
• Minimum of 1 year of experience as a physical therapist in a home health setting.
• Valid state licensure to practice as a physical therapist, adhering to professional standards.
• Current CPR certification required during clinical employment.
• Valid driver's license with the ability to travel to various client locations.
