Physical Therapist - Home Health | Full-Time | Waco, TX

We are seeking a dedicated and compassionate Physical Therapist (PT) to join our growing home health care team in Waco, Texas. This full-time position offers an exceptional opportunity to provide personalized, one-on-one patient care in the comfort of patients' homes. As a home health physical therapist, you'll play a vital role in helping patients regain mobility, independence, and improve their overall quality of life through customized rehabilitation programs.

Key Responsibilities

  • Conduct comprehensive in-home patient assessments and evaluations to identify rehabilitation needs and functional limitations
  • Develop and implement individualized physical therapy treatment plans tailored to each patient's specific goals and conditions
  • Provide therapeutic exercises, manual therapy techniques, gait training, and other evidence-based interventions
  • Educate patients and family members on home exercise programs, fall prevention strategies, and techniques to improve mobility and function
  • Monitor and document patient progress, making adjustments to treatment plans as needed to optimize outcomes
  • Collaborate with physicians, nurses, occupational therapists, and other healthcare professionals to ensure coordinated, comprehensive patient care
  • Maintain accurate, timely, and compliant clinical documentation in accordance with regulatory standards
  • Manage caseload efficiently while traveling to patient homes throughout the Waco area

Qualifications & Requirements

  • Current, unrestricted Physical Therapist license in the state of Texas
  • Doctor of Physical Therapy (DPT) or equivalent degree from an accredited program
  • Home health care or outpatient rehabilitation experience preferred, but new graduates welcome to apply
  • Strong clinical assessment and treatment skills across various patient populations
  • Excellent interpersonal, communication, and patient education abilities
  • Proven ability to work independently with strong time management and organizational skills
  • Valid driver's license, reliable transportation, and current auto insurance
  • CPR certification (or willingness to obtain upon hire)

Compensation & Benefits

  • Competitive annual salary: $105,000 - $115,000, commensurate with experience
