Your Day-to-Day
- Perform comprehensive physical assessments and review medical histories to accurately evaluate patient conditions.
- Develop and implement individualized treatment plans aligned with each patient's rehabilitation goals.
- Lead therapeutic exercise programs and mobility training to improve function and independence.
- Educate patients and family members on injury prevention, wellness strategies, and recovery expectations.
- Monitor patient progress and modify treatment plans as needed to optimize outcomes.
- Collaborate with physicians, nurses, and other healthcare professionals to support coordinated patient care.
- Maintain accurate, timely, and compliant patient documentation and records.
- Travel to patient homes throughout the service area to deliver high-quality physical therapy services.
What We're Looking For
- Minimum of nine months of experience as a Physical Therapist.
- Home health or community-based care experience preferred.
- Master's degree in Physical Therapy from a CAPTE-accredited physical therapy education program.
- Current Physical Therapist license in the state of agency operation and compliance with accepted professional standards and practices.
- Valid CPR certification required and maintained throughout employment.
- Valid driver's license and ability to travel to patient locations as needed.

Compensation: $101,000 – $112,000 annually
