The Opportunity

This home health RN role places you directly in the lives of patients navigating complex medical needs — conducting full nursing assessments, managing individualized care plans, and delivering skilled interventions where it matters most: at home. You'll be a key clinical anchor for patients and their families while coordinating closely with an interdisciplinary team.

Your Day-to-Day

• Perform comprehensive nursing assessments addressing physical, emotional, psychosocial, and spiritual needs
• Collaborate with the interdisciplinary team to develop, implement, and evaluate personalized plans of care
• Provide skilled nursing interventions including wound care, medication management, IV therapy, symptom and pain control, and patient/family education
• Deliver emotional support and tailored education to patients and caregivers based on individual needs
• Supervise LPNs/LVNs and home health aides while coordinating care activities and clinical documentation
• Respond to urgent patient needs, participate in admissions and on-call responsibilities, and adjust care plans based on ongoing monitoring
• Maintain accurate, compliant clinical records and deliver high-quality, compassionate care at every visit

What We're Looking For

• Graduate of an approved school of professional nursing with current RN licensure in Minnesota
• Minimum one year of experience as a registered nurse
• Valid CPR certification
• Valid driver's license, insured vehicle, and ability to travel to patient locations across the service area
