About the job Home Hemodialysis Registered Nurse (RN)

Louisville, KY | $38–$43/hr

The Opportunity

As a Home Hemodialysis RN, you'll provide chronic kidney disease patients with the clinical expertise and hands-on support they need to manage dialysis safely from home. This role puts you at the center of a care model built around patient independence — giving people with kidney disease the freedom to live on their own terms. You'll build deep, ongoing relationships with patients and families while working with meaningful autonomy across the full hemodialysis care cycle.

Why You'll Love This Role

  • Flexible scheduling and strong work-life balance
  • Deep, longitudinal patient relationships — not revolving-door care
  • Meaningful autonomy in how you manage and advance patient care plans
  • Strong job growth in a high-demand specialty
  • The direct satisfaction of empowering patients with kidney disease to live more freely

Your Day-to-Day

  • Develop and manage comprehensive nursing care plans — assessing, planning, implementing, and evaluating chronic hemodialysis treatment
  • Collaborate with physicians and the broader care team to interpret lab results, adjust treatment plans, and address patient concerns
  • Coordinate patient referrals to dieticians, social workers, and vascular access centers as clinical needs arise
  • Perform all aspects of the hemodialysis procedure in accordance with your state licensure and facility protocols
  • Maintain dialysis equipment in optimal working condition and respond to emergency procedures as needed
  • Document and report patient information accurately across all phases of care
  • Educate patients and families to promote safe, confident home dialysis management
  • Adhere to facility policies and comply with applicable state and federal regulations

What We're Looking For

  • Graduate of an accredited nursing program; BSN or health-related bachelor's degree preferred
  • Current RN licensure in the state of Kentucky
  • Minimum one year of nursing experience; hemodialysis and/or ICU/CCU experience strongly preferred
  • Completed training in the theory and practice of hemodialysis
  • Current CPR certification per American Heart Association standards
  • Valid driver's license, reliable transportation, and applicable auto insurance
  • Computer proficiency is a plus
