The Opportunity
As a home health Occupational Therapist, you'll help patients reclaim independence in their own homes — evaluating functional abilities, designing individualized treatment plans, and delivering hands-on rehabilitation focused on activities of daily living. Your work directly supports aging in place and meaningful recovery for patients with complex needs, including neurologic conditions.
Your Day-to-Day
- Assess patients' functional abilities and develop individualized treatment plans centered on ADLs and IADLs
- Implement rehabilitation techniques with a focus on complex neurologic injuries and conditions
- Teach patients and caregivers adaptive strategies, use of assistive devices, and home modifications to promote safety and independence
- Provide skilled occupational therapy interventions in the home environment
- Collaborate with interdisciplinary team members including nurses, physical therapists, and physicians
- Document care, progress, and outcomes in compliance with regulatory and agency standards
- Travel to patient homes within the designated coverage area
What We're Looking For
- Master's degree in Occupational Therapy (or equivalent) with active OT licensure or certification in Minnesota
- Minimum two years of independent clinical experience
- Specialized knowledge of rehabilitation techniques for complex neurologic conditions preferred
- Hands-on experience in home health, medical-surgical, or rehabilitation settings preferred
- Valid driver's license with the ability to travel across all service locations
