The Opportunity
This home health SLP role puts you at the forefront of patient recovery — evaluating and treating speech, language, cognitive-communication, voice, and swallowing disorders in patients' homes. You'll design individualized plans of care, provide specialized dysphagia therapy, and collaborate across disciplines to help patients regain function and independence.
Your Day-to-Day
- Evaluate and diagnose speech, language, cognitive-communication, voice, fluency, and swallowing disorders; assess home environment and identify equipment needs
- Develop and implement individualized, physician-approved plans of care with measurable therapy goals
- Provide dysphagia therapy and recommend appropriate diet modifications in coordination with the care team
- Recommend and train patients and caregivers on AAC devices and home exercise programs
- Monitor patient progress, adjust the care plan as needed, and submit accurate clinical documentation per JCAHO standards
- Collaborate with physicians, nurses, PT, OT, and other interdisciplinary team members through care conferences and joint planning
What We're Looking For
- Master's degree from an approved Speech-Language Pathology program (or as required by applicable law and regulation)
- Certificate of Clinical Competence in Speech-Language Pathology (CCC-SLP) from ASHA, or eligibility to obtain
- Minimum one year of clinical experience in Speech-Language Pathology
- Active SLP licensure or certification in Florida
- Valid CPR certification
- Valid driver's license and insured vehicle
