Your Day-to-Day

  • Partner with a collaborative, community-focused care team to coordinate patient needs
  • Utilize shared clinical systems to support seamless, high-quality care delivery
  • Serve as a consistent, accessible provider for families who depend on local healthcare

What We're Looking For

  • Board-Certified Family Nurse Practitioner (FNP-BC)
  • Master's degree in Nursing (MSN) with NP certification
  • Background in primary care, family medicine, internal medicine, community health, or outpatient practice preferred
  • Clinician who values long-term patient relationships and team-based, community-centered care
  • Open to living or relocating to Wayne County, the northern Finger Lakes, or the Rochester–Syracuse corridor
