Your Day-to-Day
- Deliver full-scope outpatient primary care to a loyal, relationship-driven Wayne County patient panel
- Manage acute and chronic conditions across all life stages, from pediatrics through geriatrics
- Partner with a collaborative, community-focused care team to coordinate patient needs
- Utilize shared clinical systems to support seamless, high-quality care delivery
- Serve as a consistent, accessible provider for families who depend on local healthcare
What We're Looking For
- Board-Certified Family Nurse Practitioner (FNP-BC)
- Master's degree in Nursing (MSN) with NP certification
- Background in primary care, family medicine, internal medicine, community health, or outpatient practice preferred
- Clinician who values long-term patient relationships and team-based, community-centered care
- Open to living or relocating to Wayne County, the northern Finger Lakes, or the Rochester–Syracuse corridor
