Key Responsibilities

• Conduct comprehensive patient assessments, including medical history review and evaluation of physical conditions
• Develop and implement individualized rehabilitation treatment plans based on patient needs and therapy goals
• Provide therapeutic exercises and interventions to improve patient mobility, strength, and functional independence
• Educate patients and family members on injury prevention, recovery techniques, and home exercise programs
• Monitor patient progress through ongoing evaluation and adjust treatment plans to optimize outcomes
• Collaborate effectively with physicians, nurses, occupational therapists, and other healthcare professionals
• Maintain accurate, timely, and compliant patient documentation and medical records
• Travel to patients' homes throughout the service area to deliver high-quality home health physical therapy services

Qualifications & Requirements

• Master's degree in Physical Therapy from an accredited program required
• Minimum of 1 year of professional experience as a Physical Therapist in a home health care setting
• Current, unrestricted Physical Therapy license in the state of Tennessee
• Valid CPR/BLS certification (must be maintained throughout employment)
• Valid driver's license with reliable transportation and ability to travel to multiple patient locations daily
• Strong clinical assessment and treatment planning skills
• Excellent communication and patient education abilities
• Commitment to delivering compassionate, patient-focused care

Compensation & Benefits

This position offers competitive hourly compensation ranging from $70 to $75 per hour, commensurate with experience and qualifications.
