Physical Therapist – Outpatient Rehab, Skilled Nursing & Assisted Living

Full-Time – Permanent or Travel | $75,000–$104,000/year

Your Day-to-Day

You'll initiate and implement personalized physical therapy programs based on physician orders, working with geriatric, subacute, and adult patient populations. Your responsibilities include:

• Maintain detailed evaluations, progress notes, and discharge summaries to track functional outcomes
• Collaborate with nursing and rehab staff to enhance residents' recovery and independence
• Contribute to interdisciplinary care planning meetings to support holistic treatment approaches
• Track patient statistics, manage treatment documentation, and request necessary supplies
• Participate in or lead staff training and education sessions
• Provide leadership support in the absence of the Director of Physical Therapy

What We're Looking For

• Graduate of an accredited Physical Therapy program
• Active New York State Physical Therapy license (required)
• Strong interest in working with geriatric, subacute, and adult patient populations
• Leadership qualities and ability to direct support staff when needed
• Previous experience preferred, but new PT grads are encouraged to apply
