  • Ensure adherence to HIPAA, OSHA, and all home health regulatory requirements
  • Coordinate care transitions and communicate effectively with physicians and referring providers

Job Requirements

  • Current, unrestricted Physical Therapist license in the state (required)
  • Minimum 1 year of home health physical therapy experience (strongly preferred)
  • New graduates with clinical home health rotations will be considered
  • Current CPR/BLS certification from an accredited provider (required)
  • Valid driver's license and reliable transportation for home visits (required)
  • Strong clinical assessment and critical thinking skills
  • Excellent verbal and written communication abilities
  • Proven ability to work independently with minimal supervision
  • Outstanding organizational skills and meticulous attention to detail
  • Proficiency with electronic medical records (EMR) and documentation systems
