As the Director System Care Coordination, you'll play a key role in shaping the future of care management across a growing system dedicated to delivering compassionate, accountable, and collaborative care.

Role Highlights:

• Reports to senior system leadership.

• Responsible for system-wide Care Coordination functions, including Utilization Review, Social Work, Navigation, and Case Management.

• Oversees care coordination across hospitals, clinics, post-acute care, and other entities within Aspirus Health.

• Leads the development and execution of strategic plans for integrated cross-continuum care management.

• Accountable for recruitment, retention, and professional development of care coordination teams.

• Integral in achieving health plan and health system quality and performance goals.

• Bachelors Degree in Nursing required; Masters Degree in nursing, healthcare administration, or business required.

• Minimum of 5 years

   in care coordination, preferably within an integrated healthcare system or health plan.

• Active RN license in Wisconsin or Michigan required upon hire.
