本人明白香港體育學院有限公司在考慮本人的上述職位申請時，可能會向本人現職及前任僱主進行諮詢。本人同意此項安排。

I understand that in considering my application for the above post, the Hong Kong Sports Institute Limited may request for references from my present and past employers listed above. I agree to this arrangement.

若申請職位的日常職務乃從事與兒童有接觸的工作，在香港體育學院有限公司要求下，本人同意進行性罪行定罪紀錄查核。

If the usual duties of the post applied for require contact with children, I agree to undergo the Sexual Conviction Record Check if requested by the Hong Kong Sports Institute Limited.

本人亦明白倘若故意虛報資料或隱瞞重要事實，即使已獲香港體育學院有限公司錄用，亦有可能遭解僱。

I also understand that if I wilfully give any false information or withhold any material information, I shall render myself liable to dismissal if I am appointed to the service of the Hong Kong Sports Institute Limited.
