About the job Home Health Manager (RN)
Position Overview
The Home Health Manager (RN) plays a vital leadership role in coordinating and managing skilled home healthcare services. This position is responsible for setting the operational framework for the home health team to ensure participants receive exceptional, compassionate care within their homes and community settings.
The manager oversees the delivery of home-based nursing, physical therapy, and occupational therapy services, as well as care provided in nursing homes and board-and-care facilities.
Key Responsibilities

Direct and manage a high-performing home care department while ensuring all workflows, policies, and compliance standards are maintained.

Recruit, train, and coach home care staff to foster excellence in service delivery and team collaboration.

Oversee and evaluate the effectiveness of nursing assessments and participation in interdisciplinary team (IDT) activities.

Ensure home care nurses provide and document nursing interventions consistent with individualized care plans.

Monitor the quality and consistency of care provided to participants across home and residential settings.
Qualifications

Active California Registered Nurse (RN) License required.

Bachelors Degree preferred; equivalent experience may be considered.

Minimum of two (2) years of nursing experience in a home health or related setting, preferably with a frail or elderly population.

Strong clinical assessment and care coordination skills for complex patient cases.
