Key Responsibilities:

  • Conduct assessments to identify clients' mental health needs and develop personalized care plans.
  • Provide emotional support, crisis intervention, and case management for clients.
  • Facilitate access to community resources such as housing, healthcare, and employment support.
  • Collaborate with mental health professionals, social workers, and community organizations to coordinate services.
  • Educate clients and families about mental health conditions, coping strategies, and available services.
  • Monitor client progress and adjust care plans as necessary.
  • Advocate for client rights and reduce barriers to accessing mental health services.
  • Organize and participate in community outreach and mental health awareness initiatives.

Qualifications:

  • Bachelors degree in Social Work, Psychology, Counseling, or a related field.
  • Certification in Mental Health First Aid or equivalent is preferred.
  • Minimum of 2 years of experience in community mental health or social services.
  • Knowledge of community resources and mental health services.
