Provide ongoing review of the agency's patient medical records and perform daily functions needed to ensure the agency can continue to provide timely care and billing by ensuring orders and needed documents are filed timely within the medical record, along with fulfilling electronic supply orders that are placed by referral partners or clinicians for the agency's patients.

The main responsibilities of a MEDICAL RECORDS COORDINATOR include:

  • Responsible for placing calls, receiving/sending electronic faxes, review, and scan documents, and uploading files into the electronic medical records system.

  • Coordinate and respond to medical record requests that come into the agency

  • Send Physician orders and track for signature and track Face-to-Face encounter documentation.

  • Lead initiatives to contact physicians and marketing staff to timely obtain orders, addendums, and other documents appropriately signed; prioritize unsigned orders older than 30 days.

  • Process all physician notifications timely, according to workflow and regulations.

  • Receive, process, and upload all medical record documents timely and in accordance with standard operating procedures, to ensure a complete patient medical record.

  • Prepare and distribute medical records, according to policy, to comply with payor requests, medical record reviews, medical record requests, and pre-/post- payment documentation requests.

  • Prepare admission and recertification packets as needed.

  • Upload admission consents into patient profile.

  • Assist with medical supplies and prepare car stock orders.

  • Review and approve Medline orders.

  • Approve and submit Byram and SG Homecare orders.

  • Submit medical supply orders through Cardinal Health for Kaiser patients

  • Maintain patient supply reports and submit to supervisor.

  • Provides educational opportunities and in-services to home health employees to meet quality standards, agency policy and all regulatory standards.

  • Responsible for creating an environment that is conducive to achieving professional goals among the employees through training and educational resources.

  • Assist field staff with Durable Medical Equipment (DME) orders.

  • Follow up on patient transfers.

  • Participates in Admin/Clinical Management team meetings as requested.

  • Demonstrates evidence of continued professional development.

  • Completes other assignments as requested and assigned.

  • May have access to personal health information ("PHI") necessary to fulfill the above duties and responsibilities.  Access to use and ability to disclose PHI is further defined by each organization/department.
