Position Overview: We are seeking a dedicated and compassionate PRN Home Health Registered Nurse (RN) to join our team. The ideal candidate will be responsible for providing skilled nursing care to patients in their homes, ensuring the delivery of high-quality care in accordance with physician orders and established care plans.

Key Responsibilities:

  1. Assessment and Care Planning:

    • Conduct comprehensive assessments of patients' health status and develop individualized care plans in collaboration with the interdisciplinary team.
    • Regularly evaluate patients' progress and revise care plans as needed to address changing needs and goals.
  2. Skilled Nursing Care:

    • Administer skilled nursing care, including wound care, medication management, IV therapy, and other specialized treatments as prescribed by the physician.
    • Educate patients and their families on disease management, medication regimens, and self-care techniques to promote optimal health outcomes.
  3. Patient Advocacy:

    • Serve as an advocate for patients, ensuring their needs and preferences are communicated and addressed within the care plan.
    • Collaborate with other healthcare professionals, community resources, and support services to coordinate comprehensive care and support services for patients.
  4. Documentation and Reporting:

    • Maintain accurate and timely documentation of all nursing interventions, assessments, and patient responses in the electronic medical record system.
    • Communicate effectively with the interdisciplinary team, providing updates on patient status, changes in condition, and response to treatment.
  5. Quality Assurance and Compliance:

    • Adhere to regulatory standards, agency policies, and best practices in home healthcare to ensure compliance with state and federal regulations.
    • Participate in quality improvement initiatives, clinical audits, and performance improvement activities to enhance the quality of care delivery.

Qualifications:

  • Current licensure as a Registered Nurse (RN) in the state of practice.
  • Minimum of one year of clinical nursing experience, preferably in home health or a related setting.
  • Strong assessment, critical thinking, and problem-solving skills.
  • Excellent communication and interpersonal skills, with the ability to interact effectively with patients, families, and healthcare team members.
  • Proficiency in electronic medical record documentation and basic computer skills.
  • Valid driver's license and reliable transportation.
  • CPR certification (BLS) preferred.

Work Schedule: This is a PRN (as-needed) position with flexible scheduling options based on patient needs and staffing availability.

Physical Demands:

  • Ability to lift and move patients and equipment as needed.
  • Frequent travel to patients' homes within the designated service area.
