At Home Harmony is seeking a Dementia Navigator to join our innovative GUIDE Program, a collaborative care model designed to enhance the dementia care experience for patients and their families. In this highly impactful, non-clinical role, you'll serve as a trusted guide—helping families navigate the complexities of dementia with clarity, confidence, and support.
What You'll Do
As a Dementia Navigator, you will be the consistent, compassionate point of contact for patients and caregivers throughout their journey. You will:
- Build meaningful relationships with participants and caregivers as their primary non-clinical contact
- Provide education on dementia progression, caregiving strategies, and safety planning
- Welcome and onboard new participants, ensuring a smooth and supportive start to the program
- Conduct monthly check-ins to assess needs, offer guidance, and identify changes in condition or caregiver stress
- Collaborate closely with Nurse Navigators, Advanced Practice Providers, and interdisciplinary team members
- Coordinate access to community resources, including respite care and support services
- Reinforce care plans to ensure participants feel confident implementing recommendations
- Accurately document interactions and updates within program systems
- Participate in team meetings, case reviews, and ongoing program development
- Ensure compliance with CMS GUIDE program standards and organizational policies
What You Bring
Required:
- High school diploma or equivalent (Bachelor's degree preferred)
- Experience in dementia care, caregiving, social services, or a related field
- Strong understanding of Alzheimer's disease and related dementias
- Exceptional communication and interpersonal skills
- A compassionate, patient-centered approach to supporting families
- Strong organizational skills and attention to detail
- Comfort using technology and documentation systems
Preferred:
- Certification or formal training in dementia care (e.g., Certified Dementia Practitioner)
- Experience in community-based programs, senior services, or care coordination
How You'll Work
- Full-time position
- Onsite during initial onboarding and training

- 4 days onsite
- 1 day remote
