Referral Coordinator

Launched in 2022, At Home Harmony is an innovative home-based Medical and Pharmacy practice based in Richmond, VA . We provide exceptional primary care for aging, homebound adults with complex chronic conditions. 

At Home Harmony is seeking an enthusiastic and organized Referral Coordinator to support our growing practice. This dual-role position is critical in ensuring seamless care coordination and building strong community partnerships. The ideal candidate will be friendly, outgoing, and detail-oriented, able to manage both relationship-building and the coordination of referrals with efficiency and professionalism.

Referral Management:

This portion of the role will account for approximately 70% of the roles overall responsibilities.

• Receive and process referrals submitted by providers, ensuring they are accurately directed to the appropriate referral partner or specialist.
• Confirm receipt and acceptance of referrals by the designated partner.
• Coordinate appointments for patients and ensure they are aware of upcoming visits with referred providers.
• Follow up to confirm appointment completion and obtain corresponding clinical documentation or visit notes.
• Ensure clinical documentation from referral partners is uploaded into the patients chart and routed to the appropriate provider for review.
• Maintain accurate records of all referral activity and assist with referral tracking and documentation.

Community and Partner Relations:

This portion of the role will account for approximately 30% of the roles overall responsibilities.

• Serve as a liaison between At Home Harmony and local healthcare partners including home health agencies, hospice providers, specialty offices, and hospital systems.
• Build and maintain strong, positive relationships with external partners through regular communication and engagement.
• Plan, coordinate, and host in-service events or educational sessions at At Home Harmony to introduce new or existing resources to our clinical team.
• Represent the practice in a professional and friendly manner in all interactions with external stakeholders.
• Identify new partnership opportunities that align with the needs of our patients and providers.

Clinical Duties & Responsibilities

• Work with the AHH interdisciplinary team of providers and nurse staff to provide ongoing chronic care management assistance, primarily focused on outbound referrals but also for holistic, clinical needs.

• Complete concise, clear, timely documentation in the EMR and supporting software to meet documentation requirements.

• Promote At Home Harmony's Coordinated Model of Care of Medical and Pharmacy

• Track and analyze data, including both incoming referrals from partners and outgoing referrals to external provider

Job Requirements

• 5 years' experience in at least one of the following settings: case management, disease management, home health care, hospital or outpatient office setting

• Experience in Geriatrics, Family Practice, Internal Medicine, Cardiac or Pulmonary with a passion for working with the Geriatric population

• Current unrestricted Virginia Nursing License, required

• Current BLS, required

• 2 years electronic medical record (EMR) experience Athena EMR, preferred

• Knowledge of chronic care monitoring/management, preferred

• Strong technology skills use of various systems

• Strong, professional oral and written communication skills

• Outgoing, adaptable and friendly personality

• Motivated and flexible to changing needs of the program, team and work environment, with ability to self-direct in certain prioritization of tasks

• Ability to efficiently work remotely and continue to meet productivity metrics

• Knowledge of physician office practice operations, preferred

• Valid drivers license with clean driving record

• Ability to lift and carry equipment up to 50 pounds

• Ability to walk up and down several flights of stairs easily

• Ability to work remotely and support providers that are working outside the greater-Richmond area
