You will manage a smaller panel of our most complex patients, supported by an exceptional care team, with the time and tools you need to deliver meaningful care.

Your Role:

As a physician with At Home Harmony, you will serve in three primary capacities: 

1. Medical Directorship (33%) - Partner with APPs to elevate care and consult on escalated cases. - Provide mentorship and clinical oversight to clinical team as appropriate. - Collaborate with up to 6 APPs as needed per VA practice guidelines for quality chart review. - Assist with clinical workflows and contribute medical expertise to strengthen team-based care delivery.

2. Complex Patient Care (ACO-Aligned) (33%)

- Deliver direct medical care to some of our most complex patients via a combination of in-home visits and telehealth. - Manage chronic conditions, perform comprehensive assessments, and ensure adherence to ACO quality standards.

- Lead end-of-life and goals-of-care discussions with compassion. - Coordinate care with families, medical decision makers, and other providers.

3. Chronic Care Management (CCM) Initiating Provider (33%) - Perform initiating visits for patients entering CCM. - Draft comprehensive care plans, in-line with CMS requirements.

Experience and Qualifications:

  • Board-certified MD/DO in Geriatrics, Palliative, Family Medicine, or Internal Medicine, with at least three years of experience.
  • Passion for working with seniors; home-based care experience is a plus.
  • ACO experience is a plus.
  • Active, unrestricted VA medical license and DEA certificate.
  • No history of license or controlled substance disciplinary actions and not under current investigation.

Schedule and Compensation:

  • Full-timeWork four days per week with a blend of telehealth, admin and in-home visits. 
  • No nights! No weekends! 
  • Part-time options - as little as two days a week! 
  • Competitive salary range- Base plus bonus and equity opportunity, commensurate with experience.

This position will report to the COO.
