At Home Harmony is an innovative, rapidly growing medical and pharmacy practice transforming healthcare for seniors in the comfort of their homes. We're seeking an experienced Primary Care, Internal Medicine, or Geriatric Physician to serve as a clinical leader on our collaborative care team.

What Makes This Opportunity Unique?

Rather than managing a demanding daily patient schedule, you'll focus on clinical leadership, collaboration, and improving patient outcomes. You'll oversee and support a team of 3–4 experienced Nurse Practitioners and Physician Assistants, providing clinical guidance and expertise to ensure exceptional care for patients with complex chronic conditions.

Your role will include:

• Providing clinical oversight, consultation, and mentorship to APPs
• Reviewing complex cases and developing collaborative treatment plans
• Conducting limited patient visits when physician-level care is needed
• Participating in interdisciplinary care coordination and quality initiatives
• Utilizing innovative technology, including AI-assisted documentation and remote patient monitoring

This is a primarily office-based position with limited patient visits, allowing you to positively influence the care of a much larger patient population.

Qualifications

• MD/DO with at least 3 years of experience in Family Medicine, Internal Medicine, or Geriatrics
• Passion for caring for older adults with complex medical needs
• Strong clinical judgment and collaborative leadership skills
• Active, unrestricted medical license and DEA registration in good standing
• Home-based care experience is a plus, but not required
