Position Summary:

We are seeking a full-time, onsite Registered Nurse (RN) to join our team in Petersburg, VA as our Director of Home Care. The Director of Home Care will oversee client care, conduct assessments, ensure regulatory compliance, and support our team of 45 personal care aides. This position plays a critical role in quality assurance, client advocacy, and supporting our agency's mission to provide exceptional home care services.

Key Responsibilities:

  • Conduct initial and ongoing nursing assessments for personal care clients.
  • Develop and update individualized care plans in accordance with Medicaid requirements.
  • Supervise personal care aides and conduct routine supervisory visits.
  • Ensure compliance with Virginia Department of Health (VDH), Department of Medical Assistance Services (DMAS), and other regulatory bodies.
  • Complete documentation accurately and timely, in accordance with agency policy and state regulations.
  • Serve as a liaison with physicians, case managers, clients, and families to ensure quality and continuity of care.
  • Participate in quality assurance and improvement activities.
  • Train and mentor staff as needed on clinical protocols, infection control, and best practices.

Qualifications:

  • Current active RN license in the Commonwealth of Virginia (required).
  • Minimum of 2 years RN and management experience, preferably with Medicaid patients or in a home care/personal care setting.
  • Strong knowledge of Virginia Department of Health (VDH) regulations, DMAS policies, and home care compliance requirements.
  • Excellent verbal and written communication skills.
  • Valid Virginia drivers license and reliable transportation for local travel.
  • Strong organizational and documentation skills.
  • Ability to work independently and as part of a team.

Preferred Qualifications:

  • Experience with electronic medical records (EMR).
  • Familiarity with EVV (Electronic Visit Verification) systems.

Compensation and Benefits:
