To help us continue to provide outstanding services, we are seeking a dedicated Care Coordinator to join our team.

Role Purpose

The Care Coordinator will be responsible for managing and coordinating care packages, ensuring staff are effectively deployed, and maintaining compliance with CQC standards. This role is key to ensuring service users receive safe, consistent, and high-quality care tailored to their individual needs.

Key Responsibilities

  • Coordinate care services in line with individual care plans, ensuring continuity and quality.

  • Develop, review, and update care plans with input from service users, families, and professionals.

  • Schedule and allocate staff to meet care package requirements, ensuring safe staffing levels.

  • Support the Registered Manager in meeting CQC compliance and regulatory requirements.

  • Monitor service delivery, carry out audits, and address any issues promptly.

  • Act as a point of contact for service users, families, staff, and external professionals.

  • Support with staff training, supervision, and competency checks where needed.

  • Participate in on-call rota to support service continuity.

Person Specification

Essential

  • Experience in coordinating or delivering care services within a CQC-regulated environment.

  • Strong knowledge of care planning, safeguarding, and compliance frameworks.

  • Excellent organisational and scheduling skills.

  • Strong communication and interpersonal skills with the ability to build positive relationships.

  • Ability to work under pressure and adapt to changing priorities.

  • IT literacy, including care management systems.

Desirable

  • NVQ Level 3 in Health & Social Care (or working towards).

  • Previous experience in a care coordination role.

  • Experience supporting individuals with mental health needs, physical disabilities, or complex care.
