RN Director Case Management

Hiring: Director of Case Management Inpatient Rehabilitation Hospital
Location: Richmond, VA
Reports to: Chief Executive Officer
Salary Range: $95,000 $115,000 annually, based on experience

We are actively recruiting for a Director of Case Management to lead the case management team at a 40-bed inpatient rehabilitation hospital with an average daily census of 38. This is a leadership-level position that plays a key role in driving quality outcomes, discharge planning, and coordination of care.

Key Responsibilities:

• Oversee and manage all functions of the Case Management department
• Provide leadership, support, and direction to a team of 8 direct reports (3 full-time and 5 PRN Case Managers; mix of RN and Social Work professionals)
• Ensure effective coordination of patient care and discharge planning without carrying a personal caseload
• Collaborate closely with physicians, nursing, therapy, and external agencies to promote quality, cost-effective care
• Maintain regulatory compliance and support performance improvement initiatives

Qualifications:

• Licensed RN (Registered Nurse) or SW/MSW (Social Worker with Master's degree) required
• LPNs will not be considered
• Must have prior acute care hospital leadership experience in Case Management
• Professional demeanor with excellent communication and customer service skills
• Certification in Case Management (CCM or ACM) preferred; if not already certified, must be willing to obtain certification within the first year of employment

Work Schedule:
This is a full-time, salaried position typically working Monday through Friday, 8:00 AM 5:00 PM. As a department director, flexibility is essential must be available after hours or weekends as needed to support departmental or patient care needs.
