We are seeking an exceptional, highly qualified Director of Case Management & Social Services to lead and oversee the strategy, function, and operations of our Case Management and Social Work Department. This role is integral to delivering the highest quality, clinically appropriate care to patients, while ensuring the most cost-effective use of the hospital's resources. You will be responsible for leading population-focused teams to drive exceptional clinical outcomes, while fostering an environment of excellence in service and financial performance.
As the Director of Case Management & Social Services, you will provide leadership, guidance, and strategic direction to department managers and staff. Your leadership will ensure quality, operational efficiency, and the achievement of strategic objectives in alignment with our mission. You will be responsible for fostering an engaged and competent team, ensuring adherence to regulatory standards, and overseeing the budget and operational decisions within your department.
Key Responsibilities:
Leadership & Staff Development
- Lead and implement strategic initiatives and high-level projects to ensure departmental and organizational objectives are met.
- Oversee staff development to meet quality, service, and cost-effectiveness goals.
- Ensure employee engagement through guidance, coaching, and ongoing feedback.
- Drive retention and satisfaction through effective team management, including staffing, performance evaluations, and regular communication.
- Establish and maintain effective working relationships across departments to ensure consistent care delivery and collaborative efforts.
Operational Excellence & Service Delivery
- Manage department operations and projects to ensure goals are accomplished within defined time frames and standards.
- Foster a culture of accountability to drive customer/patient satisfaction and meet key performance metrics.
- Ensure department strategies are aligned with overall healthcare trends, supporting clinical quality and patient outcomes, including length of stay optimization and effective discharge planning.
- Educate physicians and team members on best practices related to discharge planning and care progression.
Quality & Safety
- Ensure a safe and compliant working environment, meeting all regulatory and accreditation guidelines.
- Collaborate with interdisciplinary teams to address patient care challenges, ensuring safe and efficient care delivery, reducing readmissions, and improving mortality indices.
- Lead initiatives to enhance departmental safety and operational workflows, leveraging tools like Lean principles.
- Monitor compliance with policies and regulations, ensuring staff credentials and training are up to date.
Financial Stewardship
- Develop and manage department budgets, ensuring resources are allocated effectively and in line with financial goals.
- Optimize productivity and efficiency, reducing unnecessary overtime and resource consumption.
- Lead efforts in utilization review, identifying opportunities to improve resource management and avoid denials.
Growth & Innovation
- Drive innovation in care management practices, staying current with emerging trends in care coordination and evidence-based practice.
- Collaborate with community providers to address social determinants of health and manage population health.
- Lead change initiatives, adapting quickly to new responsibilities and environments while maintaining operational efficiency.
Required Qualifications:
- Education:
- Masters degree required, or must be obtained within 2 years. Must be currently enrolled in a graduate program at time of hire.
- Experience:
- Minimum of 5 years of people management experience in a healthcare setting (internal employees: 4 years required). Proven leadership experience with a focus on driving performance in healthcare environments.
- Licenses & Certifications:
- RN - Registered Nurse - Texas State Licensure (or Compact State Licensure) within 60 days, or
- LCSW - Licensed Clinical Social Worker - State Licensure within 1 year, or
- LMSW - Licensed Medical Social Worker - State Licensure within 1 year,
- Certified Case Manager (CCM) or Accredited Case Manager (ACM) or similar certifications within 1 year.
Knowledge, Skills, and Abilities:
- Strong leadership and critical thinking skills, with a proven ability to manage teams, build consensus, and drive performance.
- Extensive knowledge of regulatory and accreditation requirements impacting healthcare services.
- Excellent written and verbal communication skills, with the ability to engage with diverse stakeholders.
- Proficient in Microsoft Office Suite; experience with InterQual preferred.
- Ability to work under pressure, manage competing priorities, and lead teams effectively.
- Strong negotiation and problem-solving skills to facilitate collaboration across departments.
- High level of analytical skills with the ability to develop and implement solutions that improve outcomes.
