RN Clinical Hospice Liaison 

Role Overview

A registered nurse–based liaison role focused on building and maintaining relationships with healthcare providers, hospitals, and care facilities to increase hospice referrals and educate staff, patients, and families about hospice services. Combines clinical expertise with outreach, communication, and relationship-building.

Schedule & Location

  • Monday to Friday, 8:30 AM – 5:30 PM
  • Phoenix, Arizona (field-based hospital and facility visits)

Key Responsibilities

  • Develop and maintain strong relationships with physicians, hospital staff, case managers, and care facilities
  • Conduct regular outreach visits to generate and manage hospice referrals
  • Promote hospice services and ensure understanding of care options among healthcare partners
  • Support patient eligibility discussions using clinical knowledge
  • Meet with patients and families to explain hospice philosophy and obtain care consent
  • Contribute to growth goals by increasing census and referral volume

Required Qualifications

  • Bachelor's degree in Nursing or related healthcare field (preferred)
  • Active, unrestricted Registered Nurse (RN) license
  • At least 1 year of healthcare sales or marketing experience
  • 1–2 years of hospice or palliative care experience (strongly preferred)
  • Strong relationship-building and communication skills
  • Valid driver's license, reliable transportation, and insurance (travel required)
