Reliance Hospice and Palliative Care is seeking a compassionate Full-Time Licensed Vocational Nurse (LVN) to join our hospice team. This is a field-based position covering the San Fernando Valley area.

Schedule & Compensation

  • Position: Monday to Friday
  • Weekend coverage: Candidates willing to cover weekends as needed will be strongly considered
  • Rate: $35/hour or more, depending on experience
  • Position type: Field-based role – candidates must be able to drive within assigned coverage areas

Responsibilities

  • Provide direct nursing care to hospice patients in private homes, assisted living facilities, board and care homes, and skilled nursing facilities under RN supervision
  • Perform and document nursing interventions in accordance with the Plan of Care
  • Assist with patient assessments, orders, and coordination of interdisciplinary visits
  • Administer medications and treatments as ordered
  • Monitor patient condition and report changes to the RN
  • Educate patients and families on care needs and symptom management
  • Triage calls as needed and attend death visits
  • Maintain accurate, timely documentation
  • Preserve patient dignity and comfort at end of life
  • Make scheduled, requested, and Continuous Care visits as required

Experience Requirements

  • Experience working with Older adults, chronically or seriously ill patient, Hospice, home care, or community health settings
  • Background in caregiving, case management support, or patient advocacy preferred
  • Volunteer hospice or healthcare experience considered

Qualifications

  • Active California LVN license (required)
  • Minimum 1 year of hospice or home health experience (required)
  • Current CPR certification
  • Valid California driver's license and proof of auto insurance
  • Strong communication, documentation, and organizational skills
  • Ability to work independently in home and community settings
  • Empathy, emotional resilience, and comfort discussing sensitive topics
  • Phlebotomy and/or IV certification (preferred)
  • Multilingual candidates encouraged to apply

Physical Requirements

  • Ability to walk or stand for extended periods
  • Ability to lift and transfer patients and equipment
  • Frequent bending, kneeling, reaching, and crouching
  • Adequate vision and hearing to perform LVN duties safely
