Reliance Hospice and Palliative Care is seeking a compassionate Full-Time Licensed Vocational Nurse (LVN) to join our hospice team. This is a field-based position covering the San Fernando Valley area.
Schedule & Compensation
- Position: Monday to Friday
- Weekend coverage: Candidates willing to cover weekends as needed will be strongly considered
- Rate: $35/hour or more, depending on experience
- Position type: Field-based role – candidates must be able to drive within assigned coverage areas
Responsibilities
- Provide direct nursing care to hospice patients in private homes, assisted living facilities, board and care homes, and skilled nursing facilities under RN supervision
- Perform and document nursing interventions in accordance with the Plan of Care
- Assist with patient assessments, orders, and coordination of interdisciplinary visits
- Administer medications and treatments as ordered
- Monitor patient condition and report changes to the RN
- Educate patients and families on care needs and symptom management
- Triage calls as needed and attend death visits
- Maintain accurate, timely documentation
- Preserve patient dignity and comfort at end of life
- Make scheduled, requested, and Continuous Care visits as required
Experience Requirements
- Experience working with Older adults, chronically or seriously ill patient, Hospice, home care, or community health settings
- Background in caregiving, case management support, or patient advocacy preferred
- Volunteer hospice or healthcare experience considered
Qualifications
- Active California LVN license (required)
- Minimum 1 year of hospice or home health experience (required)
- Current CPR certification
- Valid California driver's license and proof of auto insurance
- Strong communication, documentation, and organizational skills
- Ability to work independently in home and community settings
- Empathy, emotional resilience, and comfort discussing sensitive topics
- Phlebotomy and/or IV certification (preferred)
- Multilingual candidates encouraged to apply
Physical Requirements
- Ability to walk or stand for extended periods
- Ability to lift and transfer patients and equipment
- Frequent bending, kneeling, reaching, and crouching
- Adequate vision and hearing to perform LVN duties safely
