Holy Family Medical Center in Des Plaines, Illinois is currently seeking a compassionate and experienced Hospice Nurse Practitioner to join our care team at Prime Healthcare Illinois Medical Group. This full-time, day-shift position is focused on delivering exceptional, patient-centered care to individuals and families facing advanced illness in a supportive, interdisciplinary setting.

Position Details

  • Full-time, day-shift employment
  • Provide comprehensive hospice and palliative care services
  • Collaborate with physicians, nurses, social workers, and spiritual care providers
  • Conduct patient assessments and develop individualized care plans
  • Manage symptoms such as pain, anxiety, and shortness of breath
  • Communicate effectively with patients and families regarding prognosis, treatment options, and end-of-life decisions
  • Ensure timely and accurate documentation in compliance with regulatory standards

Qualifications

  • Licensed Nurse Practitioner in the state of Illinois
  • Certification in Hospice and Palliative Nursing preferred
  • Strong communication, empathy, and clinical decision-making skills
  • Ability to work both independently and collaboratively within an interdisciplinary care team

A reasonable compensation estimate for this role, which includes estimated wages, benefits, and other forms of compensation, is $120,000-$125,000 on an annualized basis. The exact starting compensation to be offered will be determined at the time of selecting an applicant for hire, in which a wide range of factors will be considered, including but not limited to, skillset, years of applicable experience, education, credentials and licensure.
