Primary Care APP (Near-Northwest Side / Wicker Park)

Saint Mary of Nazareth Hospital in Chicago, IL, is currently seeking an experienced and compassionate Advanced Practice Provider (Nurse Practitioner or Physician Assistant) to join our team at our outpatient Primary Care clinic in near-northwest side/ Wicker Park.

• Prime Location: Practice in a walkable, energetic corridor with boutique shops, acclaimed restaurants, craft breweries, street art, live music, and cultural festivals. Enjoy easy access to the Blue line, quick commutes downtown, and the perfect blend of city energy and neighborhood charm.
• Practice Setting: Outpatient clinic environment, with seamless integration to hospital-based specialists, diagnostics, and ancillary services for coordinated patient care.
• Patient Population: Serve a vibrant, diverse community with a mix of preventive care, chronic disease management, acute visits, women's health, and minor procedures.
• Collaborative Model: Work closely with board-certified physicians, multidisciplinary teams, and support staff in a supportive, team-based atmosphere focused on quality outcomes and patient satisfaction.

Position Highlights

• Will be employed by Prime Healthcare Illinois Medical Group
• Full-time | Monday-Friday, Dayshift schedule 8am-5 pm | Outpatient work setting
• Deliver full-spectrum primary care to patients of all ages
• Manage chronic conditions, perform preventative care, and address acute health concerns
• Work closely with physicians and clinical staff to ensure coordinated, high-quality care
• Maintain accurate and timely documentation in the electronic medical record

Qualifications

• Licensed NP or PA in the state of Illinois
• Certification as a Nurse Practitioner or Physician Assistant
• Experience in primary care, family medicine, internal medicine, or urgent care is highly desirable
• Bilingual skills (Spanish or Polish) a plus, given our diverse patient population

A reasonable compensation estimate for this role, which includes estimated wages, benefits, and other forms of compensation, is $110,000.00 to $125,000.00 on an annualized basis. The exact starting compensation to be offered will be determined at the time of selecting an applicant for hire, in which a wide range of factors will be considered, including but not limited to, skillset, years of applicable experience, education, credentials and licensure.
