About the job Home Health Intake Specialist
Your Role:
The primary function of the Intake Coordinator is to facilitate the referral intake process, coordinate care with the interdisciplinary team, the patient, their family, and the referral source. If clinical, may be required to perform in-home visits.
What You Need:
Education: Graduate of an accredited school of Vocational/Practical nursing preferred. If non-clinical, must have
high school diploma or equivalent.
Licensure: If clinical, current state license as a Licensed Vocational Nurse or Licensed Practical Nurse. Multi-state
license issued by a state member of the National Nurse Licensure Compact (NLC) preferred. Current drivers
license. If required to make in-home visits, must have current CPR card.
Preferred skills/expertise
Experience: One year experience as a Licensed Vocational/Practical Nurse, homecare experience preferred.
Skills: Nursing skills as defined as generally accepted standards of practice. Good interpersonal skills. Ability to
establish and maintain effective working relationships with all segments of the staff, the lay and professional
public
