Job Title: Patient Management Coordinator

Reporting Structure: Nikita Patel, Senior Manager Clinical Operations Job Overview: The Patient Management Coordinator is the operational command center for all GLP and Advanced Medical Therapy client fl ow. This role manages handoff s between Nurse Practitioners (NPs) and studios, ensures documentation accuracy, maintains monthly program tracking, and catches issues before they reach the client. The PMC ensures the GLP workfl ow runs cleanly, consistently, and without surprises.

Key Responsibilities AMT ( Advanced Medical Therapy) Client Workfl ow Oversight Manage the end-to-end Advanced Medical Therapy client journey between NPs and studios. Ensure clients move on time through intake, telemed, prescribing, and follow-ups. Serve as the primary operational hub for all AMT client fl ow and coordination. Airtable Tracking and Monthly Management Maintain, update, and regularly audit Airtable dashboards for all GLP clients. Track monthly prescription activity, program adjustments, survey completion, and follow-up needs. Refresh dashboards with any operational or clinical updates as required. Communicate, verify, and adjust medication tiers in POW based on NP direction or client status. Proactively identify and resolve data gaps before they aff ect patient care. AMD Form Monitoring Monitor AMD daily for newly submitted forms (HHF, consents, medical history, labs). Ensure all documentation populates accurately in each client chart. Flag missing, incomplete, or incorrect forms to studios for immediate resolution. Calendly Appointment Pre-Check Review upcoming appointments to confi rm all required AMD documentation is present. Verify health history completion and required labs are uploaded. Reschedule, hold, or escalate appointments that do not meet pre-check standards. Studio and NP Communication Management Facilitate and manage communication between NPs and studios. Ensure NP guidance reaches studios clearly and quickly. Keep NPs updated on studio issues such as missing forms or client concerns. Track all open items through completion and closure. Ship-to-Home Tracking Support Provide studios with tracking support for ship-to-home GLP clients. Manually monitor shipments from partner pharmacies (CSM). Help studios set clear delivery expectations and troubleshoot shipping delays with clients. Key Metrics for Success: INP appointments proceed without delays caused by missing or incomplete documentation. Airtable and POW stay accurate, up to date, and well maintained.

Communication between NPs and studios is clear, timely, and dependable. Ship-to-home clients are tracked consistently and accurately. AMT operations run smoothly with minimal issues or bottlenecks. Skills & Requirements: Strong operational rigor with exceptional attention to detail. Strong organizational and time-management skills to manage multiple client cases and schedules effi ciently. Profi ciency in tech platforms and systems - Ability to manage complex, multi-step workfl ows across Airtable, AMD, POW, and Calendly. Excellent written and verbal communication skills are essential to interact eff ectively with diverse stakeholders including clients and internal medical teams. Proactive problem-solver who spots issues early and escalates them appropriately. Experience with EHR systems, data analytics, and telehealth platforms is highly preferred. Previous experience in patient coordination, clinical support, or medical offi ce administration required.
