Licensed Social Worker

Licensed Social Worker – North Shore, MA area including Lynn

Full-Time | Monday–Friday | Day Shift (until 4 PM)

We are seeking Licensed Social Workers (LICSW/LCSW) to support geriatric patients through a mix of infacility and patient home/facility visits.

Key Responsibilities

• Participate in IDT assessments, meetings, and care planning, 
• Complete social work assessments and maintain timely, accurate documentation.
• Provide psychosocial assessments, supportive counseling, and collaborate with health providers.
• Coordinate hospital, rehab, and nursing home admissions/discharges
• Support transitions to long‑term care and collaborate with Medicaid Specialists, SNFs, and caregivers.
• Facilitate family meetings and connect families with community resources.
• Assist with guardianship updates, health care proxy education, and insurance‑related documentation.
• Review care plans with participants, guardians, and/or activated HCPs.
• Complete authorizations for home care and other approved services.
• Conduct home and SNF visits as needed; support palliative and end‑of‑life planning.
• Communicate updates regarding disenrollment, LTC conversion, site transfers, and demographic changes.
• Report allegations of abuse and complete required documentation.

Qualifications

• Active Massachusetts LCSW or LICSW
• Experience with geriatric populations
• Strong assessment, communication, and documentation skills
• MA Driver's License
