Monday-Friday, 8 AM 4 PM | No nights, weekends, or weekends
Hybrid schedule 2 days remote, 3 days in the field/office
Salary: 73k to 90's k

What You'll Do:

• Complete in-home assessments and create care plans

• Work with providers and community partners to meet member goals

What Were Looking For:

• Active MA RN license (required)

• Nursing experience (home care, case management, or community health a plus)

• Strong communication and organization skills

• Reliable transportation for local travel
