Overview
We are seeking a compassionate and experienced Hospice Registered Nurse Case Manager to join our Cleveland-based hospice team. In this role, you will provide expert clinical care, symptom management, and emotional support to patients and families during the end-of-life journey. Working closely with the interdisciplinary team including social workers, chaplains, and hospice aides you will coordinate individualized care plans that ensure comfort, dignity, and quality of life.
Key Responsibilities
Patient Care & Case Management
- Develop and implement individualized hospice care plans addressing physical, emotional, and spiritual needs.
- Provide expert pain and symptom management, including crisis intervention when needed.
- Conduct comprehensive assessments during home visits and adjust care plans accordingly.
Family & Caregiver Support
- Educate families and caregivers on hospice care, medication administration, and comfort measures.
- Offer emotional support and anticipatory grief counseling.
Interdisciplinary Collaboration
- Partner with physicians, social workers, chaplains, and aides to ensure holistic, patient-centered care.
- Actively participate in Interdisciplinary Team (IDT) meetings to review and update care plans.
Compliance & Documentation
- Maintain accurate, timely documentation in the EMR, ensuring adherence to Medicare, Medicaid, and state hospice regulations.
- Follow all hospice best practices, agency policies, and accreditation standards.
Professional Development & Advocacy
- Mentor peers and support staff, sharing clinical expertise to strengthen the care team.
- Advocate for patients wishes, ensuring care aligns with their goals and values.
Qualifications
- Associates or Bachelors degree in Nursing (BSN preferred).
- Current RN license in the state of practice.
- Certification in Hospice and Palliative Nursing (CHPN) preferred.
- Minimum 2 years of hospice or palliative care nursing experience.
- Proficient in pain and symptom management, crisis intervention, and hospice care best practices.
- Knowledge of CMS hospice regulations and state-specific compliance requirements.
- Strong communication, assessment, and critical thinking skills.
- Proficiency with electronic medical records (EMR) systems.
Work Environment
- Provides care in patients homes, hospice facilities, and assisted living communities within the service area.
